Your Wealth Report

Welcome to your Wealth Report questionnaire. Over the next few pages you'll tell us about your NHS service and pay, anything you hold outside the NHS, and the income you'd like in retirement. Please allow around 10 minutes to complete the form.

What you'll need

Please have the following to hand before you start if possible. They're not essential; however, having all this information means we can deliver the most accurate report we can.

your latest NHS Pension Annual Benefit Statement, from Total Reward Statements
a recent payslip, for your band and pay point
when you first joined the NHS Pension Scheme
your State Pension forecast
the latest statements for any pensions or savings outside the NHS
a rough idea of the income you'd like in retirement

Your answers and your data

Your answers save in this browser as you go, so you can finish later on the same device; on a shared computer, please use a private window. When you submit, we email you a copy: reply to it if anything needs correcting. We use your answers only to build and invoice your report, never sell them, and delete them within 90 days of your request, or sooner if you ask. Our privacy notice sets out the details.

Payment

Your report costs £49. No payment is taken here: once you submit, we email you a copy of your answers, then an invoice. Your invoice is due within 30 days; if it isn't paid by then, we void it and delete your answers. We start building your report once it's paid, and send it within 5 working days of payment.

Refunds and your rights

You have a legal right to cancel within 14 days of placing your order. The questionnaire asks you to let us start building your report as soon as you've paid, and to accept that this right ends once the report is delivered. If you cancel before then, we refund you in full.

Our promise goes further: if you're not happy with your report,tell us before it arrives or within 30 days of receiving it, and we'll refund you in full. If anything in it looks unclear or incorrect, you can also get in touch and we'll look at it, and re-issue the report if needed. Our terms set out the details.
